Description
Seen from a posterior approach in an adult male, the superior suprarenal arteries arise bilaterally from the inferior phrenic arteries and course inferiorly and medially onto the superior pole of each suprarenal (adrenal) gland. The abdominal aorta lies left of midline, with the inferior vena cava positioned anterior and slightly rightward in relation to it, while the kidneys occupy the posterior abdominal wall with their upper poles tucked beneath the diaphragmatic crura. Fine arterial twigs spread across the adrenal capsule, closely related to the perirenal fat and the upper renal fascia. Posterior orientation matters here because it clarifies the proximal origin of the adrenal arterial supply at the diaphragmatic level, a point that can be obscured in anterior schematics where the gland dominates the field. During adrenalectomy, retroperitoneal exposure, or control of bleeding from small adrenal branches, surgeons often meet these vessels near the inferior phrenic artery before the central adrenal vein is addressed, and the short, friable superior suprarenal branches can retract and ooze into the periadrenal tissue. Small vessels, big nuisance. Teaching value is high for courses covering retroperitoneal anatomy, endocrine surgery, and abdominal vascular variants, and for atlas figures that need to show the relationship of the suprarenal arteries to the diaphragm, aorta, inferior vena cava, kidneys, and adjacent autonomic nerves and lymphatics.
