Description
Arising from the anterior division of the internal iliac artery, the superior vesical artery courses anteroinferiorly along the lateral pelvic wall before turning medially to reach the superolateral aspect of the urinary bladder. From a lateral perspective, its vesical branches fan across the bladder dome and upper body, running superficial to the detrusor and beneath the peritoneal reflection, with smaller arterioles tracking alongside accompanying vesical veins. Posterior and inferior to this vessel, the ureter typically descends toward the bladder base, while the iliopsoas mass and adjacent pelvic fascia form a muscular and fascial backdrop lateral to the vascular pedicle. Relation to the obturator neurovascular bundle and the lumbosacral trunk is often appreciable in this angle. Clear orientation matters. For pelvic surgeons, this view maps the arterial supply that is commonly encountered during bladder mobilization and anterior pelvic dissection, including steps in radical prostatectomy, cystotomy, and pelvic lymph node dissection. Ligation or traction injury to superior vesical branches can produce troublesome bleeding at the bladder dome, and preservation becomes relevant in reconstructive work where bladder vascularity is a limiting factor. The proximity of the superior vesical artery to the distal ureter and the internal iliac system also makes it a useful landmark when teaching the internal iliac branching pattern and the safe planes on the lateral pelvic wall. Urology and pelvic anatomy courses use this plate to teach internal iliac artery branching, vesical vascular territories, and relationships that explain hematuria sources and operative bleeding. It also fits well in surgical atlases, resident handouts, and patient education material describing bladder and prostate procedures in the male pelvis.
