Description
Oriented in an anterior pelvic view of an adult male, the urinary bladder sits midline and superior to the prostate gland, with small-bowel loops and a segment of colon draped more superiorly in the abdominal cavity. The inferior vesical artery is traced along the posteroinferior aspect of the bladder base, giving off a prostatic branch that courses inferomedially to enter the prostatic capsule and distribute through the glandular parenchyma. Venous channels accompany the arterial pattern, consistent with the vesical and prostatic venous plexuses, while fine neural elements suggest the pelvic autonomic nerves running in close proximity to these vessels. Attention to the prostatic branch matters when you are thinking about bleeding control and tissue viability in transurethral resection of the prostate and open or robotic simple prostatectomy, where capsular and periurethral branches can be encountered unexpectedly. Arterial input to the prostate also underlies targeted embolization strategies for benign prostatic hyperplasia, in which variant origins from the inferior vesical artery, internal pudendal artery, or obturator artery can change catheter selection and raise the risk of nontarget embolization to the bladder, rectum, or penis.
