Oriented from an inferior perspective, the posterior surface of the brainstem and posterior cranial fossa contents are presented with the vertebral arteries approaching the foramen magnum and giving rise to a posterior meningeal branch that turns toward the dura. The vessel courses posteriorly and laterally along the margins of the posterior fossa, in close relationship to the medulla oblongata and adjacent to the dural reflections overlying the cerebellar hemispheres. Cranial nerves emerge as paired bundles from the ventral brainstem and pontomedullary junction, their roots lying medial and anterior to the meningeal arterial twig as it leaves the parent vertebral artery.
