Description
Arising from the popliteal artery in the posterior knee, the inferior medial genicular artery courses medially along the inferior margin of the medial femoral condyle toward the proximal tibia, passing deep to the tendinous expansions of semimembranosus and adjacent to the medial head of gastrocnemius. Distal to the joint line it contributes to the genicular anastomosis, with fine branches tracking along the medial tibial condyle and toward the anterior tibial recurrent network. Posteriorly, the popliteal vein and smaller venae comitantes accompany the arterial tree, while the tibial nerve runs superficially in the popliteal fossa before descending between the heads of gastrocnemius. A clean medial angle clarifies the relationship of vessel to capsule and collateral ligament complex. For surgeons working around the posteromedial corner, this artery matters. During high tibial osteotomy, medial meniscal root repair, or posteromedial arthroscopic portal placement, the inferior medial genicular artery lies in the corridor where capsular release and suture passage occur, and iatrogenic injury can produce brisk bleeding or postoperative hemarthrosis. In total knee arthroplasty and revision cases, its contribution to periarticular circulation also explains why aggressive medial soft tissue stripping can compromise capsular perfusion and delay wound healing. Small vessel, real consequences.
