Description
Obliquely from the lateral aspect of the male pelvis, the urinary bladder fills the field with the superior vesical artery coursing along its superolateral surface and sending multiple branches over the outer curvature toward the dome. Proximally, the artery is consistent with an anterior division branch of the internal iliac artery, running anterior and inferior before breaking into smaller arterioles that penetrate the detrusor. A ureter approaches the posterolateral bladder wall toward the trigone, while the urethra continues inferiorly from the bladder neck; a rounded glandular structure inferior to the base suggests the prostate in approximate position. Arterial and venous vessels are color differentiated, clarifying inflow and drainage on the bladder surface. A lateral view of the superior vesical artery matters when you need to understand what gets sacrificed, or preserved, during pelvic surgery. Ligation or thermal injury to vesical branches can contribute to bladder wall ischemia, impaired healing, or postoperative hematuria after procedures such as radical prostatectomy, cystotomy, or pelvic lymph node dissection.
