Description
Centered in the pelvis, the male urinary bladder is shown from an anterior perspective with the superior vesical artery ascending on the anterosuperior surface toward the bladder dome. Proximally, the vessel arises from the patent portion of the umbilical artery off the anterior division of the internal iliac artery, then gives small branches that course medially across the detrusor. Bilateral ureters enter posterolaterally at the bladder base (trigonal region not directly faced in this view), while the urethra descends inferiorly from the bladder neck. Fine autonomic fibers accompany the vessels, with sympathetic and parasympathetic contributions converging as the vesical plexus along the lateral bladder wall. Small, named branches matter here. For pelvic surgeons, this orientation clarifies where superior vesical branches run when the peritoneum is reflected and the space of Retzius is opened, a common step during open or laparoscopic cystotomy, suprapubic catheter placement, and radical prostatectomy. Misidentification of the umbilical artery remnant versus a patent superior vesical artery can lead to troublesome bleeding along the anterior bladder wall. The juxtaposition of vascular supply with bladder innervation also supports teaching of neurogenic bladder, where pelvic splanchnic (S2 to S4) injury alters detrusor contraction while vascular anatomy remains intact. Urology and pelvic anatomy courses can use this plate to anchor discussions of internal iliac artery branching patterns, vesical plexus distribution, and safe zones for anterior bladder entry.
