Description
Arising from the anterior surface of the abdominal aorta just inferior to the celiac trunk, the superior mesenteric artery (SMA) descends anterior to the left renal vein and then crosses the third (horizontal) part of the duodenum before entering the root of the mesentery. From this anterior perspective, proximal branches are logically visible, including the inferior pancreaticoduodenal artery, multiple jejunal and ileal arteries coursing within mesenteric folds, and the ileocolic, right colic, and middle colic arteries tracking toward the right lower quadrant and transverse mesocolon. Companion veins run alongside the arterial tree, converging toward the superior mesenteric vein, and the superior mesenteric plexus is suggested by yellow neural elements around the SMA origin. Lymph nodes line the mesenteric vessels in green along the mesenteric root. That relationship to the third part of the duodenum is the key teaching point. The SMA can compress the duodenum against the aorta in superior mesenteric artery syndrome, classically after rapid weight loss with loss of the mesenteric fat pad, producing postprandial pain and bilious vomiting. Surgeons also care about the SMA and its colic branches during right hemicolectomy and complete mesocolic excision, where ligation level influences lymphadenectomy and risks ischemia at the marginal artery of Drummond.
