Description
Posterior cervical and upper thoracic back musculature is rendered with the iliocostalis cervicis isolated within the lateral column of the erector spinae. Its muscular slips rise superomedially from the angles of the upper ribs (classically ribs 3 to 6) to insert on the posterior tubercles of the transverse processes of C4 to C6, lying lateral to the longissimus cervicis and lateral and slightly posterior to the cervical vertebral laminae and spinous processes. Superficial layers such as trapezius, with adjacent deltoid contour laterally and latissimus dorsi inferiorly, frame the deeper paraspinal anatomy while fascia and tendinous intersections emphasize the segmented appearance of the muscle belly. Segment-by-segment depiction matters because iliocostalis cervicis is often discussed as a single strip, yet clinicians palpate and needle it as discrete bands along the posterolateral neck and upper thorax. Trigger points in the iliocostalis group can refer pain to the occiput and posterior shoulder and can mimic cervical facet-mediated pain, while asymmetric activation contributes to cervical side-bending and rotation patterns seen after whiplash and sustained forward-head posture. Clear placement relative to the transverse processes and rib angles also supports teaching safe trajectories for dry needling and for interpreting focal edema or fatty infiltration on cervical and upper thoracic MRI. Small muscle, common culprit.
