Description
Centered in the anterior mediastinum, the heart sits slightly left of the midline, with the right atrium and right ventricle forming most of its anterior (sternocostal) surface and the left ventricle projecting inferolaterally toward the apex. Superiorly, the ascending aorta continues into the aortic arch, while the pulmonary trunk lies anterior to the aorta before bifurcating into right and left pulmonary arteries. The superior vena cava enters the right atrium from above, and the inferior vena cava ascends from the abdomen to the right atrium inferiorly; superficial coronary vessels course within the coronary (atrioventricular) groove and the anterior interventricular sulcus. Adipose tissue thickens the epicardial contours. An anterior close-up like this gives a clean read on vessel relationships that matter in practice: the pulmonary trunk’s position anterior and leftward relative to the ascending aorta, and the right atrium’s proximity to the cavoatrial junction where central venous catheters terminate. Obesity adds a familiar clinical layer, increased epicardial fat along the coronary grooves can obscure coronary arteries on echocardiography and correlates with coronary artery disease risk, and a larger cardiomediastinal silhouette can complicate plain-film interpretation when screening for cardiomegaly or pericardial effusion.
