Description
Arising from the abdominal aorta at the pelvic brim, the right and left common iliac arteries descend inferolaterally and divide into internal and external iliac arteries anterior to the sacroiliac region. Medial branches of the internal iliac system course into the lesser pelvis toward the urinary bladder and prostate, while lateral branches track along the pelvic sidewall toward the gluteal region. Parallel venous channels accompany these arteries, converging as the internal and external iliac veins and reuniting superiorly as the common iliac veins to form the inferior vena cava. An anterior perspective matters in the male pelvis because most operative and interventional navigation begins from the front, where the iliac bifurcation, pelvic brim, and inguinal region define access corridors. This view helps you anticipate high-risk zones for hemorrhage during pelvic fracture fixation, lymphadenectomy, or hernia repair, where the external iliac vessels lie just deep to the inguinal ligament and the internal iliac branches disappear quickly into the pelvis. It also supports teaching of venous thrombosis patterns, with iliofemoral DVT propagation into the common iliac veins and the potential for compression syndromes at the pelvic brim.
