Description
Posterior surface anatomy is paired with an internal, anatomically positioned urinary tract in a female child, with the right and left kidneys (renes) lying retroperitoneally on either side of the vertebral column. Each renal pelvis narrows into a ureter that descends inferiorly along the posterior abdominal wall toward the pelvis, maintaining a bilateral, near-paravertebral course before curving anteromedially toward the urinary bladder (vesica urinaria). The kidneys sit superior to the iliac crests and lateral to the spinous processes, while the ureters track inferiorly toward the midline. Paired symmetry is clear. For pediatric teaching, a posterior view clarifies a common point of confusion: the kidneys are not low in the pelvis, and renal pain often localizes to the flank and costovertebral angle rather than the midline back. That relationship matters when discussing pyelonephritis, vesicoureteral reflux with secondary hydronephrosis, and ureteropelvic junction obstruction, where dilation follows the ureter’s superior-to-inferior path and can be mentally mapped onto the child’s body surface. It also supports procedural planning, since posterior access to the collecting system in percutaneous nephrostomy is oriented to the kidney’s retroperitoneal position and rib-adjacent upper pole.
