Description
Sectioned cranial bone exposes the cranial cavity and highlights the basilar venous plexus as a reticular channel system on the clivus, lying posterior to the sphenoidal sinus region and anterior to the brainstem compartment. Superiorly, the plexus communicates with the cavernous sinus complex along the body of the sphenoid, while inferiorly it approaches the margin of the foramen magnum and the upper internal vertebral venous plexus. Adjacent dural venous sinuses are represented in continuity, including the superior sagittal sinus along the midline calvaria and transverse and sigmoid sinuses coursing posterolaterally toward the jugular foramina. Orientation is clear. Anatomists and surgeons care about this area because the basilar plexus forms a valveless intercavernous and vertebrobasilar connection that can redirect venous outflow when jugular drainage is impeded, and it provides a route for cranial, facial, and spinal venous communication. Its position on the clivus places it directly in the operative corridor for endoscopic endonasal approaches to the clivus and petroclival region, where venous bleeding can obscure landmarks and complicate hemostasis. The same connections explain how infection or thrombosis can propagate between cavernous sinus pathways and the vertebral venous system, a teaching point that lands well when the clivus and foramen magnum are both visible in one cut section.
