Description
Cut away at the calvaria, the cranial cavity is opened to expose the dural venous sinuses as they course along the inner table of the skull. The superior petrosal sinus runs posterolaterally along the superior border of the petrous part of the temporal bone, seated at the attachment line of the tentorium cerebelli between the cavernous sinus anteriorly and the transverse or sigmoid sinus posteriorly. Nearby landmarks that typically remain in view include the superior sagittal sinus in the midline, the confluence of sinuses and transverse sinuses along the occipital groove, and the petrous ridge forming a sharp boundary between the middle and posterior cranial fossae. Bone matters here. For teaching and operative planning, this angle clarifies why the superior petrosal sinus is a frequent bleeding source during approaches that mobilize the tentorium or work along the petrous ridge, including petroclival meningioma exposure and combined petrosal routes. Venous drainage patterns in this region vary, and the sinus may accept petrosal (superior petrosal) cerebellar veins, so injury can carry more consequence than its size suggests.
