Description
Centered on the petrous part of the temporal bone, the superior petrosal sinus runs along the superior margin of the petrous ridge, seated within the attached margin of the tentorium cerebelli and tracking from the posterior cavernous sinus toward the transverse sinus at the transverse-sigmoid junction. Medial to it lie the dorsum sellae and clivus, while the internal acoustic meatus and posterior surface of the petrous bone sit more inferolateral. Blue-coded dural venous channels outline the broader cranial base drainage, with the transverse sinuses sweeping laterally and posteriorly and the sigmoid sinuses curving inferiorly toward the jugular foramina. The foramen magnum anchors the posterior midline. For operative anatomy, the superior petrosal sinus is the venous landmark that often complicates tentorial and petroclival approaches, because it tethers the tentorium and bleeds briskly when divided near the petrosal ridge. This relationship matters in combined petrosal, retrosigmoid, and anterior petrosectomy corridors used for petroclival meningiomas, trigeminal schwannomas, and select basilar apex lesions, where control of the tentorial edge and preservation of deep venous egress can influence postoperative temporal lobe or cerebellar venous congestion. Small structure, big consequences. Neuroanatomy and neurosurgery courses use this kind of skull base venous map to teach dural sinus continuity (cavernous sinus to transverse sinus) and to orient trainees to bony landmarks such as the petrous ridge, jugular foramen, and foramen magnum during cadaveric dissection. It also fits well in atlases and operative manuals discussing tentorial incision planning, venous bleeding control, and complication avoidance in skull base surgery.
