Description
Superior sagittal sinus runs in the midline along the attached margin of the falx cerebri, seated in the sagittal groove of the calvaria and extending posteriorly toward the confluence of sinuses. From an endocranial, skull base oriented view, the transverse sinuses course laterally within the grooves of the occipital bone and temporal bone, then curve inferiorly as the sigmoid sinuses toward the jugular foramina and internal jugular veins. Bony landmarks that anchor the spatial anatomy include the anterior, middle, and posterior cranial fossae, the petrous ridges, and the foramen magnum positioned inferior and posterior in the field. Dural venous channels sit against bone, not within brain parenchyma. This perspective matters when teaching venous drainage patterns and the dural reflections that create these sinuses, because their rigid, endothelium-lined walls make them behave differently from cortical veins during trauma and surgery. Superior sagittal sinus injury is a known hazard in parasagittal craniotomy and vertex trauma, where bridging veins enter the sinus and can tear, producing acute subdural hematoma. Skull base relationships also guide interpretation of venous sinus thrombosis on CT venography or MR venography, where the transverse and sigmoid sinuses are common sites of flow defects that can mimic normal anatomic dominance.
