Description
Along the midline of the cranial vault, the superior sagittal sinus occupies the attached margin of the falx cerebri, running in the sagittal plane between the paired cerebral hemispheres. In cross section, its triangular lumen sits immediately deep to the calvaria at the sagittal suture, with the dura mater forming its rigid walls and the arachnoid and cortical surface lying inferiorly. Lateral venous channels from bridging veins approach the sinus from both sides, traversing the subdural space to enter its lateral wall, while arachnoid granulations commonly project into the lumen along its superior aspect. Midline anatomy. Clean relationships. This perspective matters because the superior sagittal sinus is the key dural venous conduit encountered in midline cranial surgery, and small errors in appreciating its dural attachments can turn a routine craniotomy into uncontrollable venous bleeding. Trauma-related tearing of bridging veins where they meet the sinus underlies many acute subdural hematomas, and the cross-sectional geometry helps explain why even modest shear forces can avulse those veins. The same anatomic corridor also frames superior sagittal sinus thrombosis, where impaired drainage produces parasagittal cortical venous congestion and hemorrhagic venous infarcts.
