Description

Entering the right lower quadrant from an anterior abdominal perspective, the ileocolic vein is traced within the mesentery as it courses with the ileocolic artery toward the superior mesenteric vein. Tributaries from the terminal ileum, cecum, and proximal ascending colon converge medially, while the cecum and ileocecal junction sit inferior and lateral to the vascular pedicle. Mesenteric lymph nodes and accompanying lymphatic channels cluster along the same vascular arcade. Small venules and arterial branches ramify toward the bowel wall. Surgical orientation to this vein matters during appendectomy and right hemicolectomy, where the ileocolic vessels form the dominant right colic pedicle and guide mesenteric division. Central vascular ligation requires clear identification of the ileocolic vein as it approaches the superior mesenteric vein, because variant venous drainage and close proximity to the superior mesenteric artery can turn a routine dissection into brisk hemorrhage. This is a reliable roadmap for mesenteric planes.
