Description
Centered over the anterior aspect of the fifth lumbar vertebra, the superior hypogastric plexus forms a broad, flattened autonomic nerve network in the retroperitoneum, immediately inferior to the aortic bifurcation and anterior to the vertebral body. Paired lumbar splanchnic nerves converge toward this plexus from a posterolateral direction, while the right and left hypogastric nerves descend inferomedially into the pelvis. The abdominal aorta and inferior vena cava run vertically in the midline, with the common iliac vessels diverging inferiorly and serving as key vascular landmarks for orienting the plexus. Bright yellow nerve fibers contrast against red arterial and blue venous channels, with the sigmoid colon and adjacent mesentery providing anterior visceral context in the male abdomen. Surgical exposure of the L5 promontory, aortic bifurcation, and proximal common iliac vessels puts this plexus at risk. One wrong plane. Injury to the superior hypogastric plexus during anterior lumbar interbody fusion (ALIF) or presacral approaches can contribute to sympathetic dysfunction, including retrograde ejaculation in men, and it also matters during oncologic and vascular dissection around the distal aorta and iliac bifurcation. This anterior perspective clarifies why meticulous midline and paramedian retraction, as well as awareness of the hypogastric nerves as they descend into the pelvis, can reduce neurologic complications. Ideal for teaching pelvic autonomic pathways in gross anatomy and neuroanatomy curricula, and for illustrating operative risk discussions in spine surgery, vascular surgery, and colorectal or urologic pelvic procedures where autonomic preservation is a priority.
