Description

Posterior lower-leg anatomy is rendered with emphasis on the crural fascia (deep fascia of the leg) forming an investing sleeve around the calf. Superficial to deeper, the gastrocnemius (medial and lateral heads) overlies the soleus, with the plantaris tendon, when present, tracking inferomedially toward the calcaneus. Inferiorly, the gastrocnemius and soleus converge into the calcaneal (Achilles) tendon, which inserts on the posterior surface of the calcaneus; proximally, the popliteal region and distal hamstring tendons approach the posterior knee, with the biceps femoris positioned laterally and the semitendinosus and semimembranosus more medially. Attention to the fascia matters in the calf because it defines the posterior compartment and its septal boundaries, the same spaces that swell in exertional compartment syndrome and threaten perfusion and nerve function. This posterior perspective also supports teaching of Achilles tendinopathy and rupture, where the tendon’s twist and paratenon relationship to the surrounding deep fascia influence pain patterns and surgical exposure, and it clarifies why a tight crural fascia can limit muscle expansion during plantarflexion.
