Description
Posterior lower legs are presented with the crural fascia (deep fascia of the leg) draped as a connective tissue sheath around the calf, blending proximally with the fascia lata and investing the gastrocnemius and soleus that dominate the superficial posterior compartment. Inferiorly, the fascia condenses into aponeurotic septa around the triceps surae and converges on the calcaneal (Achilles) tendon, which descends in the midline to insert on the posterior surface of the calcaneus. Along the deep margin, the tibia lies medial and the fibula lateral, providing fixed attachment for intermuscular septa that separate posterior from lateral and anterior compartments. A tight, continuous envelope. Crural fascia matters because its limited distensibility sets the pressure environment for the leg compartments, so edema or hemorrhage after tibial fracture, reperfusion, or vigorous exertion can precipitate acute compartment syndrome with pain on passive stretch and threatened perfusion. The same fascial planes guide surgical decompression: two-incision fasciotomy relies on knowing where the septa lie relative to the tibial crest and fibula to release the anterior, lateral, superficial posterior, and deep posterior compartments. Posteriorly, the relationship between the investing fascia, the paratenon of the Achilles tendon, and the gastrocnemius-soleus complex also frames teaching around Achilles tendinopathy, rupture, and the surgical approach for tendon repair.
