Description
Superficial and deep crural fascia envelop the male lower leg from the level of the knee to the malleoli, blending proximally with the fascia lata and distally thickening around the ankle to form retinacular bands. Along the anterior compartment, the aponeurotic sheath lies superficial to tibialis anterior and extensor digitorum longus, closely applied to the anteromedial tibial border, while laterally it spans toward the fibula and the intermuscular septa. Posteriorly, the fascia invests gastrocnemius and soleus, narrowing into the paratenon and fascial coverings adjacent to the calcaneal (Achilles) tendon as it courses to the posterior calcaneus. Bony landmarks including the tibia, fibula, and the tarsals and metatarsals of the foot provide firm attachment points and define the fascial compartments. Crural fascia matters because it is the container that makes compartment physiology real. When edema, hemorrhage, or reperfusion swelling raises pressure within the anterior or deep posterior compartments, the relatively noncompliant fascial sheath limits expansion and contributes to acute compartment syndrome, with ischemic risk to structures such as the deep fibular (peroneal) nerve and anterior tibial artery. The same fascial planes guide surgical decompression, where fasciotomy incisions are placed with respect to the tibial crest, intermuscular septa, and the course of the great saphenous vein and superficial fibular nerve.
