Seen from a posterior perspective, the arterial tree arises from the thoracic aorta and continues as the descending aorta along the left side of the vertebral column before passing through the diaphragm to become the abdominal aorta. Paired subclavian arteries course laterally toward the upper limbs, while the common iliac arteries diverge inferiorly at the aortic bifurcation to supply the pelvis and lower extremities. Cervical vessels are suggested in the neck, with the major trunks positioned medial to the scapular regions and lateral branches tracking toward the axillae. Small-caliber branches and arterioles are rendered as a tapering peripheral network. Posterior mapping of the vasculature clarifies relationships that are easy to lose in anterior heart-centric diagrams, especially the long, fixed course of the descending aorta and its proximity to the thoracic vertebrae. That spatial context matters in traumatic aortic injury at the isthmus, classically associated with rapid deceleration, and in thoracoabdominal aortic aneurysm planning where intercostal and lumbar segmental arteries contribute to spinal cord perfusion via radiculomedullary pathways. Collateral routes around aortoiliac occlusive disease also read well from behind, following the paraspinal and pelvic arterial channels. Landmarks are clear.
