Description
Seen in lateral profile beneath the facial skin, the buccinator occupies the muscular wall of the cheek, spanning from the alveolar processes of the maxilla and mandible near the molars toward the modiolus at the oral commissure. Its fibers run predominantly horizontal and lie deep to the more posterior, vertically oriented masseter on the lateral surface of the mandibular ramus. Superiorly the muscle approaches the maxillary vestibule and inferiorly it blends with fibers of orbicularis oris, forming a continuous sling around the oral cavity. Bone landmarks include the zygomatic arch, maxilla, mandible, and adjacent dentition. A clear landmark. Clinically, the buccinator matters because it compresses the cheek against the teeth to keep the food bolus on the occlusal surfaces during mastication, so weakness can present as pocketing of food in the buccal sulcus even when the tongue and jaw muscles are intact. Its surface relationship to the parotid duct (which pierces the buccinator to open opposite the maxillary second molar) makes this view useful when teaching sialadenitis, duct stones, and iatrogenic injury during cheek laceration repair. The buccal branches of the facial nerve course superficially in the cheek, while odontogenic infection can track into the buccal space, so understanding the layered anatomy helps explain swelling patterns and drainage routes.
