Buccinator Muscle Beneath the Skin

Layered beneath the facial skin, the buccinator forms a thin muscular sheet spanning from the alveolar processes of the maxilla and mandible near the molar teeth toward the orbicularis oris at the oral commissure. Laterally, the masseter occupies the ramus of the mandible as a thicker muscle of mastication, while the buccinator sits more anterior and medial, shaping the cheek wall of the oral cavity. Superiorly the zygomatic bone and maxilla frame the midface, and inferiorly the mandible and cervical vertebrae anchor the head and neck alignment. Bone and teeth provide fixed landmarks. Cheek mechanics matter. The buccinator compresses the vestibule against the dentition to keep the food bolus on the occlusal surfaces during chewing, and its relationship to the molars explains why patients with buccinator weakness from facial nerve (CN VII) palsy often pocket food in the lateral oral sulcus. Clinically, this is also the corridor for the parotid (Stensen) duct as it turns medially to pierce the buccinator opposite the maxillary second molar, a small but unforgiving landmark during intraoral incisions, sialendoscopy planning, or repair of buccal lacerations.
