Description
Centered in the plantar aspect of the forefoot, the deep plantar arch forms a transverse arterial loop deep to the plantar aponeurosis and adjacent to the bases of the metatarsals. The arch is fed primarily by the lateral plantar artery and completed by the deep plantar branch of the dorsalis pedis, then gives rise to the plantar metatarsal arteries that course distally in the intermetatarsal spaces toward the toes. Red arterial channels are paired with blue venous pathways running in parallel, tracking proximal return from the plantar venous network into the deep veins of the leg. Proximally, the tibial vessels descend in the lower leg to supply the hindfoot and midfoot before contributing to the plantar circulation. Understanding the deep plantar arch matters when you need to explain how the foot maintains perfusion despite segmental disease, because the plantar arch provides a key collateral link between the anterior tibial (dorsalis pedis) and posterior tibial (lateral plantar) systems. Distal ischemia in peripheral artery disease often declares itself in the toes, and the patency of the plantar metatarsal arteries and their digital branches influences wound healing after minor trauma or diabetic foot ulceration. This view also maps the territory at risk during plantar incisions and forefoot procedures, where injury to the deep arch or its perforating branches can produce troublesome bleeding or compromise distal flow.
