Seen from the plantar aspect of an adult male foot, the plantar digital arteries course distally along the medial and lateral margins of the toes, paired with companion veins and small arterioles in the subcutaneous plane of the sole. Proximally, these digital vessels arise from the plantar metatarsal arteries and the superficial and deep plantar arches, themselves fed by the lateral plantar artery with contribution from the dorsalis pedis via the deep plantar branch. Between the vascular channels, intrinsic plantar muscles occupy the central compartments, with the flexor digitorum brevis and quadratus plantae positioned superficial to the flexor digitorum longus tendons as they track toward the phalanges. Clear distal branching. A plantar vascular map matters most at the level of the forefoot and toes, where tissue perfusion can fail early and in a focal pattern. Ischemic toe pain, nonhealing digital ulcers in diabetes, and necrosis after embolic disease often correspond to compromise of specific proper plantar digital arteries rather than a generalized reduction in flow.
