Description
Posterior neck anatomy is opened to emphasize the deep cervical artery coursing superiorly along the posterior cervical spine toward the occipital region. From this dorsal perspective, the vessel is seen deep to trapezius and the splenius capitis and splenius cervicis, traveling in close company with the semispinalis capitis and other transversospinalis muscles that hug the laminae and spinous processes. Cervical vertebrae provide the midline scaffold, with the artery running paramedian and segmental branches angling laterally into the deep paraspinal compartment. Scapular outlines and the upper thoracic midline anchor the inferior field. Clinically, the deep cervical artery, a typical branch of the costocervical trunk, matters because it supplies the deep posterior neck musculature and forms anastomoses with the occipital artery and branches around the suboccipital region and posterior triangle. Its course between muscular layers makes it a source of persistent bleeding in posterior cervical approaches, and it can be encountered during deep dissection for cervical laminectomy exposure, posterior instrumentation, or drainage of deep neck infection tracking along the prevertebral and paraspinal spaces. Small-caliber but consequential. The posterior view also clarifies why blunt trauma or iatrogenic injury in the paraspinal gutter may produce a deep hematoma that is not immediately obvious superficially.
