Description
Dominating the inferior aspect of the thoracic cavity, the diaphragm forms paired domes with a central tendon, its right hemidiaphragm rising superior to the left over the liver. Muscular fibers sweep medially from the costal margins and lower ribs, from the xiphoid region anteriorly, and from the lumbar vertebrae via the right and left crura posteriorly, converging on the central tendon. The caval opening for the inferior vena cava lies in the right portion of the tendon, while the esophageal hiatus pierces the muscular part just left of midline, anterior to the aortic hiatus and the vertebral column. Respiratory mechanics are easiest to teach here because you can follow how contraction lowers the domes, increases thoracic volume, and simultaneously elevates intra-abdominal pressure, a combination that also influences venous return through the inferior vena cava. Herniation and reflux are not abstract concepts in this region: the relationship of the esophagus to the right crus and phrenoesophageal ligament frames the anatomy behind sliding hiatal hernia and GERD, and it also explains why diaphragmatic pinch can complicate transhiatal esophagectomy or mobilization of the distal esophagus.
