Description
Beneath the male thoracic integument, the domed diaphragm spans the inferior aperture of the rib cage, attaching peripherally to the internal surfaces of ribs 7 to 12, the xiphoid process, and via right and left crura to the anterolateral lumbar vertebrae. An anterior, slightly left lateral perspective places the sternum and costal cartilages anterior, the vertebral column posterior and medial, and the left scapula resting posterolaterally over the ribs. The central tendon sits superior to the upper abdominal viscera and inferior to the heart and lung bases, while the muscular costal fibers sweep medially toward it from the lower thoracic wall. Clinically, this relationship explains why diaphragmatic dysfunction rarely presents as an isolated structure problem and instead shows up as altered mechanics at the thoracoabdominal junction, from basal atelectasis to reduced inspiratory capacity. The view supports teaching of the major apertures, the caval opening in the right tendon, the esophageal hiatus within the right crus, and the aortic hiatus posterior to the crura, all common reference points when discussing hiatal hernia, reflux disease, and transdiaphragmatic spread of infection. Phrenic nerve (C3 to C5) motor supply is the takeaway. Irritation can refer pain to the shoulder.
