Description
Seen from a posterior perspective under the skin, the diaphragm forms a broad musculotendinous sheet spanning the inferior thoracic aperture, its right dome rising slightly superior to the left as it arches beneath the lower ribs. Costal fibers attach along the inner surfaces of ribs 7 to 12 and their costal cartilages, while the lumbar part anchors medially on the right and left crura along the anterolateral aspects of the upper lumbar vertebrae. Posteriorly, the diaphragm lies anterior to the thoracic and lumbar vertebral column and medial to the posterolateral rib cage, with adjacent intercostal musculature and the scapulae framing the upper field. The central tendon sits more anterior and is only indirectly appreciated from behind. Right left asymmetry matters. The elevated right hemidiaphragm reflects hepatic support and becomes a key reference when assessing eventration or phrenic nerve palsy, where unilateral elevation on chest radiography often correlates with basal atelectasis and exertional dyspnea. Posterior anatomy also maps directly to common pain and weakness patterns in lower thoracic surgery, since incisions near the costophrenic recess and lower intercostal spaces can irritate diaphragmatic attachments and alter breathing mechanics. A frequent blind spot for learners is how far caudal the diaphragmatic origins sit relative to the apparent thoracic cavity.
