Description
Posterior to the thoracic cage, the dorsal branches of the posterior intercostal veins course medially from each intercostal space toward the vertebral column, running deep to the posterior ribs and converging near the costovertebral region. Each vein tracks along the inferior border of its corresponding rib, then turns toward the paravertebral venous pathways adjacent to the thoracic vertebral bodies. Superiorly, venous channels continue into the lower cervical region near the shoulder girdle, while laterally the branches lie under the posterior arcs of the ribs. Bony landmarks anchor the view. Clinical teaching around the posterior intercostal veins usually centers on where they drain and how they communicate, since these segmental veins feed into the azygos and hemiazygos systems and link to the valveless vertebral venous plexuses. That connection matters in posterior thoracic surgery and spine work: segmental veins can bleed briskly when stripped from the costovertebral gutter, and the vertebral plexus provides a pathway for hematogenous spread of infection or metastasis to the thoracic spine. A slight elevation from the back makes the paravertebral convergence point easier to read against the ribs and spinous processes.
