Description
Beneath the reflected skin of the male buttock, the gluteus minimus lies deep to the gluteus medius on the lateral surface of the ilium, with its fibers running inferolaterally to a tendon inserting on the anterior facet of the greater trochanter. Posteriorly, the sacrum and lower lumbar vertebrae frame the midline, while the iliac crest sits superior and the acetabulum forms the lateral socket of the hip joint. Anterior to the muscle belly, the hip joint capsule hugs the femoral head and neck, placing the minimus tendon and the capsular fibers in close apposition near the anterosuperior joint line. The proximal femur is shown inferior to the acetabulum in anatomical alignment. Hip pain localization often hinges on distinguishing periarticular tendinopathy from true intra-articular disease, and the gluteus minimus is a frequent pain generator in greater trochanteric pain syndrome, sometimes misread as lumbar radiculopathy. Its intimate relationship to the capsule also matters in surgical planning, because the direct anterior and anterolateral approaches to total hip arthroplasty develop planes near the minimus and can irritate the tendon or denervate fibers supplied by the superior gluteal nerve.
