Description
Deep to gluteus medius, the gluteus minimus occupies the lateral surface of the ilium, arising between the anterior and inferior gluteal lines and coursing inferolaterally toward the proximal femur. A posterior view of the male pelvis and proximal thighs places its fan-shaped fibers lateral to the sacrum and posterior iliac crest, with the muscle belly hugging the outer pelvis just superior to the acetabular rim. Distally, its tendon converges to insert on the anterior facet of the greater trochanter, blending with the hip joint capsule, while the adjacent bony landmarks include the sacrum, iliac wings, and proximal femora. The superior gluteal neurovascular bundle runs on the deep surface between gluteus medius and minimus. Gluteus minimus matters because it is not just a smaller medius, it directly tensions the anterior capsule and helps control femoral head translation during gait, especially in single-leg stance and terminal swing. Denervation or tears (often alongside gluteus medius pathology) produce lateral hip pain and abductor weakness with a Trendelenburg pattern, and the anterior trochanteric insertion helps explain why MRI and surgical reports localize lesions to the anterior and lateral facets rather than the posterior trochanter.
