Description
Posterolaterally along the lumbar spine, the iliocostalis lumborum is presented as the lateral column of the erector spinae group, lying lateral to longissimus thoracis and superficial to the deep intrinsic back muscles. Its muscle belly arises from the iliac crest and the thoracolumbar fascia, then courses superiorly with obliquely oriented fibers to form tendinous slips that attach to the angles of the lower ribs, typically ribs 6 through 12. Semi-transparent skin and subcutaneous tissue remain superficial, preserving the surface topography while keeping the paraspinal contours readable in a rear three-quarter view. Fiber direction is clear. Clinically, the iliocostalis lumborum often localizes pain in lumbar strain syndromes and in myofascial trigger point patterns that refer to the sacroiliac region and posterolateral hip. That lateral relationship to the rib angles and iliac crest also matters when interpreting posterolateral thoracolumbar incisions or ultrasound-guided injections, where you need to distinguish the erector spinae plane from more superficial subcutaneous layers and from the deeper multifidus and quadratus lumborum. Palpation and needling landmarks are easier to teach from an oblique posterior vantage than from a strict midline posterior view.
