Description
Running longitudinally on either side of the lumbar spinous processes, the iliocostalis lumborum is highlighted as the lateral column of the erector spinae, positioned lateral to longissimus and continuous inferiorly toward the posterior iliac crest and thoracolumbar fascia. From this posterior, subcutaneous perspective, its muscle belly sits deep to the more superficial latissimus dorsi and the superficial fascia, tracking superiorly toward its rib attachments. The paired muscle masses frame the midline groove over the vertebral column, with fibers oriented in parallel bands that mirror the lumbar curvature. Clear bilateral symmetry. Palpating and treating low back pain often blurs the boundaries between erector spinae components, and isolating iliocostalis lumborum helps clarify what clinicians mean by a “paraspinal strain” versus involvement of quadratus lumborum or the thoracolumbar fascia. This view is also the one you want when teaching how the erector spinae contributes to lumbar extension and ipsilateral lateral flexion, with the iliocostalis functioning as the more lateral force vector on the rib cage and lumbar spine.
