Description
Within the female breast, lobules of the mammary gland and their lactiferous ducts sit embedded in the inner adipose tissue, with fibrous suspensory ligaments (Cooper ligaments) extending from the dermis toward the deep fascia. Deep to the gland and fat, the pectoralis major forms the posterior muscular bed, separated by the retromammary space that permits limited mobility of the breast over the chest wall. Arterial and venous branches course through the parenchyma (classically from the internal thoracic and lateral thoracic systems), while superficial and deep lymphatic channels track toward the axillary region and along the parasternal pathway; cutaneous sensory fibers accompany these vessels toward the nipple areola complex. Spatially, glandular elements concentrate more centrally and posterior to the nipple, while adipose predominates around and between lobes, tapering peripherally. Focusing on the inner fat is not cosmetic anatomy, it is the substrate that shapes mammographic density and the pathways along which disease can spread. In breast carcinoma, tumor infiltration and desmoplastic reaction can tether Cooper ligaments, producing skin dimpling, and malignant cells follow lymphatics to sentinel nodes, most often in level I axillary nodes but sometimes to internal mammary nodes. Surgeons rely on these planes during mastectomy and breast conserving surgery: staying superficial to the pectoral fascia and respecting the retromammary space reduces injury to the pectoralis major and helps preserve contour. A clear map of vessels and nerves also supports planning around the nipple areola complex when ischemia or sensory loss is a concern.
