Description
Anterior thoracic anatomy is rendered with the female mammary gland reflected to emphasize the internal adipose compartment interposed among glandular lobules and lactiferous ductal elements. Deep to the breast, pectoralis major forms the posterior bed, with the retromammary space separating gland from muscle over the chest wall. Superficial sensory innervation courses from the anterior and lateral cutaneous branches of the intercostal nerves toward the nipple areola complex, while arterial and venous channels travel in parallel through the subcutaneous tissue and along the gland. Green lymphatic pathways track toward the axillary basin, the dominant drainage route from the lateral quadrants. Fat distribution drives breast contour, but it also matters surgically because adipose planes and septa (Cooper ligaments) define how the breast mobilizes during reduction mammoplasty, augmentation, and mastectomy flap elevation. Oncologic staging depends on understanding where lymphatics and vessels run relative to gland and fat, since invasive carcinoma can spread through interlobular stroma to axillary nodes, and skin edema (peau d’orange) reflects lymphatic obstruction within the superficial fat. Nerve preservation is equally practical, as injury to lateral intercostal branches during lateral dissection can compromise nipple sensation. Landmarks, not decoration.
