Description
Centered in the lumbar column, the L2 vertebra is highlighted against adjacent lumbar vertebrae and the superior sacrum, with intervertebral discs separating each vertebral body. The vertebral foramen forms a near-circular aperture posterior to the vertebral body, bordered by the pedicles laterally and the laminae posteriorly, with the spinous process projecting posteriorly in the midline. Transverse processes extend laterally from the vertebral arch, and the superior and inferior articular processes frame the zygapophysial (facet) joints with L1 above and L3 below. Relative to the lumbosacral junction, L2 sits superior and slightly anterior to the sacrum, maintaining the typical lumbar lordotic alignment. Emphasis on L2 helps when teaching how lumbar vertebral morphology supports load transmission while preserving room for the cauda equina within the vertebral canal. That circular vertebral foramen becomes clinically relevant when comparing central canal narrowing patterns, because lumbar spinal stenosis usually reflects a combination of disc bulge, facet hypertrophy, and ligamentum flavum thickening rather than a small bony canal alone. Level identification also matters for procedures, since an L2 to L3 interspace target differs from the more common L3 to L4 or L4 to L5 approach used for lumbar puncture, and wrong-level surgery remains a preventable error.
