Description
Running superficially along the lateral aspect of the ankle, the lateral malleolar vein courses posterior to the lateral malleolus of the fibula and tracks inferiorly toward the lateral margin of the foot. From there it communicates with the dorsal venous network over the metatarsals and tarsals and drains proximally toward the small saphenous vein as it ascends in the posterior compartment of the leg. Bony landmarks include the distal tibia and fibula forming the talocrural (ankle) joint, the talus and calcaneus deep to the vessel, and the metatarsals and phalanges anteriorly. Surface anatomy drives the relevance of this vessel. Lateral ankle edema, varicosities along the small saphenous territory, and superficial thrombophlebitis often declare themselves around the malleoli, and a clean understanding of the lateral malleolar venous pathway helps distinguish localized superficial disease from more proximal outflow problems. For trauma and orthopaedic care, the vein’s proximity to the lateral malleolus and the calcaneofibular ligament region matters during lateral approaches to the ankle and hindfoot, where soft tissue handling can compromise superficial venous drainage and delay wound healing.
