Lateral Perspective Of The Lateral Marginal Vein

Shown from a lateral perspective, the lateral marginal vein tracks along the outer border of the foot, beginning near the lateral side of the dorsal venous arch and coursing posterior to the base of the fifth metatarsal toward the lateral malleolus. Proximally, it continues into the small saphenous vein as it passes behind the lateral malleolus and ascends in the superficial fascia along the posterior midline of the distal leg, lying superficial to the calcaneus and the lateral aspect of the Achilles tendon. Deep to the superficial venous channel, the fibula sits lateral to the tibia, and the calcaneus, cuboid, and lateral metatarsals define the osseous contour that the vein closely follows. Arteries appear in contrast, mapping the arterial supply across the dorsum and lateral foot. A clean lateral view clarifies the relationship between superficial venous drainage and the bony landmarks used in examination and access. This is the corridor where lateral foot and ankle incisions, trauma swelling, and tight dressings can obscure or compress superficial vessels, and where varicosities along the small saphenous system often present as a palpable, tender cord behind the lateral malleolus. The vein’s continuity into the small saphenous also matters during duplex mapping and when planning cannulation or avoiding superficial venous injury during peroneal tendon or lateral ligament procedures.
