Oriented from the dorsal aspect, the male foot is rendered with the lateral marginal vein running distally along the lateral border from the hindfoot toward the fifth toe, superficial to the tarsals and metatarsals and lateral to the extensor tendon group on the dorsum. Phalanges and metatarsals form the bony scaffold beneath, with tarsal elements proximally defining the contour of the lateral midfoot. Small dorsal venules converge into the marginal channel near the dorsolateral forefoot, while deeper vascular paths are suggested by companion vessels coursing between the metatarsals. Spatially, the vein tracks lateral to the dorsum and superior to the plantar soft tissues. Dorsal-lateral venous anatomy matters when you are teaching superficial venous drainage of the foot and how it feeds into the great and small saphenous systems. The lateral marginal vein continues proximally toward the lateral dorsal venous network and typically contributes to the small saphenous vein via the dorsal venous arch, a pathway implicated in varicosities, edema patterns, and planning of venous access. A clean dorsal perspective also clarifies why incisions, external fixation pins, and arthroscopy portals placed too superficially along the lateral border can injure thin-walled superficial vessels.
