Description
Running along the medial border of the foot, the medial marginal vein begins from the dorsal venous network over the forefoot and curves around the medial aspect of the first metatarsal region toward the medial malleolus. From a medial perspective you would expect to see it superficial to the fascia, tracking anterior to the calcaneal tuberosity and alongside the longitudinal arch, with the underlying tarsals, metatarsals, and phalanges forming a clear bony scaffold. Deeper structures such as the tendon of flexor hallucis longus and the posterior tibial neurovascular bundle sit plantar and posterior to this superficial venous channel. Orientation is straightforward. Proximal flow carries it toward the great saphenous vein. For venous mapping and bedside anatomy, this vessel matters because it defines the medial superficial drainage pathway of the foot and ankle and serves as a consistent surface landmark near the medial malleolus. Cannulation attempts on the dorsomedial foot, harvesting or preserving the great saphenous vein during bypass surgery, and planning incisions for medial ankle approaches all benefit from recognizing where the medial marginal vein runs and where it commonly anastomoses with plantar venous channels. It is also a common route for superficial thrombophlebitis extending from pedal veins into the saphenous system, a pattern that can mislead examiners when medial ankle erythema is attributed only to cellulitis.
