Description
Arising along the medial plantar aspect of the foot, the medial plantar vein courses anterior to the calcaneal tuberosity and deep to the plantar aponeurosis, collecting venules from the plantar skin and intrinsic soft tissues. Distally it parallels the medial plantar artery and tracks with the medial plantar nerve toward the instep, then runs proximally and superiorly along the medial border of the sole. Proximally, it converges with the lateral plantar venous channel to form the posterior tibial vein posterior to the medial malleolus, in close relationship to the flexor retinaculum and the tendons of tibialis posterior and flexor digitorum longus. Venous valves are typically present along its course. Plantar venous anatomy matters when you are tracing sources of foot edema, varicosities, or post-traumatic swelling, and when planning flap or graft harvest from the medial plantar region where venous outflow can limit wound healing. This plantar perspective also supports teaching the relationship of the plantar veins to the tarsal tunnel, a corridor where space-occupying lesions or synovitis around the flexor tendons can compress the posterior tibial neurovascular bundle and mimic or exacerbate tarsal tunnel syndrome. Orientation is the point. Knowing what lies superficial versus deep guides both dissection and ultrasound probe placement.
