Description
Medially oriented along the plantar aspect of the male foot, the medial plantar vein courses anteriorly from the plantar venous plexus toward the tarsal tunnel, running with the medial plantar artery and adjacent to the tibial artery and venae comitantes as they pass posterior to the medial malleolus. Bony landmarks such as the calcaneus, talus, navicular, and the medial cuneiform frame the vessel’s path, with the phalanges extending distally and the longitudinal arch forming the plantar contour. Superficially, the vein sits deep to the plantar aponeurosis and plantar musculature, then becomes more proximal and posteromedial as it approaches the retromalleolar region. Arteries appear in red and accompanying veins in blue, reinforcing their paired course through the medial sole. Clear identification of the medial plantar vein matters when teaching plantar venous return and when planning procedures around the tarsal tunnel, where neurovascular crowding increases the risk of iatrogenic injury. This relationship is also clinically relevant in tibial nerve entrapment (tarsal tunnel syndrome) and in surgical exposure of the posterior tibial artery, where the venae comitantes and tributaries from the medial plantar side can complicate hemostasis.
