Seen in lateral profile, the male foot and distal leg are oriented with the fibula lateral to the tibia, the talus stacked superior to the calcaneus, and the lateral malleolus forming the distal fibular prominence at the ankle. Across the dorsum of the forefoot, dorsal metatarsal veins course proximally from the intermetatarsal spaces and converge into the dorsal venous network as they approach the ankle. Distally, these veins lie superficial to the metatarsals and extensor tendons, then track posteriorly and proximally toward the lateral aspect of the ankle where the dorsal venous arcade communicates with the lateral marginal vein. Superficial vessels dominate this field. Lateral orientation matters because it clarifies how the dorsal metatarsal veins feed the superficial venous drainage pathway toward the small saphenous system, a frequent route for reflux and varicosities along the posterolateral calf. The relationship of superficial veins to the lateral malleolus is also a practical landmark during venipuncture, cannulation, and flap planning, and it helps explain why edema or hematoma can obscure dorsal venous access after ankle sprain or immobilization.
