Description
Running along the dorsum of each toe, the dorsal digital veins arise from small venules around the nail bed and dorsal skin, then course proximally over the phalanges toward the metatarsophalangeal joints. Medially and laterally, these channels converge into dorsal metatarsal veins that lie superficial to the extensor tendons and dorsal interossei, with the metatarsals and phalanges providing clear bony orientation beneath. Proximally the network feeds the dorsal venous arch across the metatarsal bases, with tributaries tracking toward the medial marginal vein (continuing as the great saphenous vein) and the lateral marginal vein (toward the small saphenous vein). Bones anchor the map. Surface venous anatomy on the dorsum of the foot is where you see everyday clinical decisions play out, because this is a common site for cannulation, trauma-related bleeding, and postoperative swelling assessment. The dorsal venous arch and its digital tributaries also explain why superficial thrombophlebitis can present with a linear tender cord over the metatarsals, and why edema from venous insufficiency often collects around the forefoot and toes before it is obvious at the ankle. In diabetes and peripheral arterial disease, comparing the superficial venous pattern with the course of the dorsalis pedis artery helps teach why palpable pulses and skin perfusion do not always correlate with visible veins.
