Description
Running longitudinally along the medial border of the scapula, the dorsal scapular vein is traced on a posterior view of the upper back as it ascends toward the cervicothoracic junction. Overlying layers include the trapezius spanning from the occipital region and nuchal ligament down to the thoracic spinous processes, with the deltoid forming the rounded contour of the shoulder laterally. Deep to trapezius, the rhomboid major and minor lie medial to the scapula, while infraspinatus occupies the infraspinous fossa inferior to the scapular spine; the scapular spine and acromion provide palpable bony landmarks, and the proximal humerus appears lateral at the glenohumeral region. Clinically, this posterior perspective clarifies how the dorsal scapular vessels track with the dorsal scapular nerve along the medial scapular border, a relationship that matters when teaching scapular winging patterns and differentiating nerve injury from muscular dysfunction. Venous anatomy here is also a practical roadmap during posterior approaches to the upper thorax and periscapular region, where the cervicothoracic junction and the plane deep to trapezius can bleed briskly if the vessel is injured.
