Description
Running posterior to the auricle, the posterior auricular vein collects venous blood from the scalp over the mastoid and occipital regions, then descends along the posterior border of the auricle toward the upper neck. It courses superficial to the mastoid process and adjacent to the occipital belly of the occipitofrontalis, with the sternocleidomastoid forming a prominent anterolateral landmark as the vein approaches the angle of the mandible. Inferiorly, the vessel is shown draining into the external jugular vein as it crosses superficial to the sternocleidomastoid within the superficial fascia. Blue venules feeding the main channel are suggested around the retroauricular sulcus. Clear depiction of this drainage pattern matters when you are mapping superficial head and neck veins for cannulation, flap planning, or managing bleeding after retroauricular incisions. The posterior auricular vein and external jugular vein sit in the operative field for mastoidectomy approaches and postauricular exposure, where injury can lead to troublesome oozing and obscured landmarks. It also helps explain venous outflow routes that can transmit scalp infection toward deeper cervical channels, a teaching point that often gets lost when only arteries are emphasized.
