Description
Posterior to the knee joint, the popliteal vein is traced within the popliteal fossa as it courses superiorly from the confluence of the anterior and posterior tibial veins toward the adductor hiatus to become the femoral vein. Relative to the popliteal artery it lies more superficial and slightly lateral at the level of the joint line, with the artery deeper against the posterior femoral cortex and joint capsule. Distal femur and proximal tibia frame the vessel, their articular cartilage rendered over the condyles and tibial plateau; the patella is absent, consistent with a posterior perspective. Superficial venous pathways along the medial leg may be suggested, including the course that communicates with the great saphenous system via perforators, although the principal emphasis remains the deep venous channel behind the knee. Posterior anatomy of the popliteal vein matters because it is a common site for deep vein thrombosis and the key conduit interrogated with duplex ultrasound when patients present with calf swelling or pain. Relationship to the artery and tibial nerve in the fossa governs safe dissection during posterior approaches to the knee, repair of popliteal aneurysm, and exposure for trauma. A small positional change can alter which structure you meet first. That detail drives surgical planning.
